Somra is a simple tool for sharing how you're doing with your care team between visits. You answer a short set of questions and your responses go to your team. Somra doesn't diagnose or make decisions about your care, your providers do that, using your check-ins as one more way to stay informed.

Somra is provided through your practice. Depending on your plan, remote check-ins may involve a cost share, like other parts of your care. Your practice can explain what to expect for your specific coverage: give them a call and they'll walk you through it.

That's okay. Just open the app and complete your next check-in when you can. Your care team sees your history over time, so one missed day won't set you back.

Only your care team at your practice. Your responses are shared securely with the people directly involved in your care and no one else.

Your practice adds you to Somra and sends an invitation to get started. There's no public sign-up; if you'd like to use it, ask your provider whether Somra is right for you.
